The Space Between
Serenelle Living exists to bridge the space between services.
We are not the hospital. We are not the social worker. We are not the housing provider. We are the connective tissue that holds a person's care together when no single system is responsible for the whole.

A System That Works in Pieces
Why Serenelle Exists
We are not here to replace anyone. We are here to coordinate all of them.
Serenelle Living is not replacing hospitals. Not social workers. Not Marion Senior Services. Not behavioral health. We coordinate all of them — so the person never has to navigate alone.

Transitional Care Without Compromise
Stabilization
One medication stood between Jimmy and a safe return.
Prescribed at HCA — couldn't afford to fill them
Arrived at SMA — two medications they couldn't provide
West Marion tried to discharge — SMA refused the transfer
We pushed — one medication change cleared the way
The hospital didn't push. We did.

What Makes Us Different
We follow the client. We do not expect the client to navigate.
The Old Way
Each system expects the client to navigate it alone.
Handoffs are hoped for, not guaranteed.
The client is responsible for connecting services that were never designed to connect.
When something falls through, it is called "non-compliance."
The Serenelle Way
One coordinator follows the client across every system.
Handoffs are planned, confirmed, and documented.
We connect the services — the client never has to.
When something falls through, we catch it, because we are still there.
16
We follow the client. We do not expect the client to navigate.

Transitional Care Without Compromise
Vernon's Place
A stabilization program. Not just housing.
Vernon's Place bridges recovery and independence through structure, accountability, and community reconnection.
Bridge Recovery
A safe, structured environment for people transitioning out of crisis — not a permanent placement, a bridge.
Restore Independence
We help residents rebuild the daily rhythms of a self-directed life.
Create Accountability
Goal setting, daily responsibilities, and honest check-ins — because recovery without structure is fragile.
Provide Structure
Predictable routines that replace chaos with stability.
Reconnect
Employment, healthcare, and community resources — re-engaged one step at a time.
17
A stabilization program. Not just housing.

Transitional Care Without Compromise
The Accountability Model
Structure that makes independence possible.
Goal Setting
Each resident sets personal, measurable goals with their coordinator.
Employment Planning
Resume building, interview coaching, and transportation to work.
Daily Responsibilities
Predictable routines and household contributions that rebuild agency.
Benefits Navigation
We help residents access the benefits they are eligible for.
Community Integration
Reconnection with faith communities, peer groups, and civic life.
18

Appendix · Q&A
How is Serenelle Living funded?
Serenelle operates on a private-pay model supplemented by grants, partnerships, and collaborative funding. We are actively seeking grant partners and pilot funding to expand access for clients who cannot afford the full cost of care. We are not a Medicaid provider, but we coordinate with Medicaid-funded services on behalf of our clients.

Appendix · Q&A
Is Serenelle Living licensed? What about HIPAA?
Serenelle operates in compliance with Florida state requirements for the services we provide. We are HIPAA-compliant: all client data is encrypted at rest and in transit, access is role-based and audited, and every staff member signs a confidentiality agreement. We do not share protected health information without proper authorization.

How does your transportation service work?
We provide non-emergency medical transportation (NEMT) for our clients — to appointments, pharmacies, the grocery store, and community resources. Our drivers are trained, our vehicles are inspected, and we coordinate rides alongside the rest of a client's care plan so transportation is never the reason a visit is missed.

What exactly does Care Coordination include?
A dedicated coordinator assigned from day one. The coordinator manages assessments, attends key appointments, communicates with every provider, reconciles medications, manages handoffs between facilities, and stays with the client across every transition — for as long as the client needs coordination.

What is Independent Living, and how is it different from a shelter?
Independent Living — including Vernon's Place — is a structured stabilization program, not a shelter. Residents set goals, maintain daily responsibilities, engage in employment planning, and reconnect with community. The model is built to restore independence, not simply to provide a bed.

Transitional Care Without Compromise
Appendix · Q&A
How do you handle behavioral health?
We do not provide clinical behavioral health treatment. We coordinate it. Our coordinators identify behavioral health needs during assessment, connect clients to licensed providers, support voluntary admissions, coordinate medication management, and stay with the client through the full course of treatment — including the transitions in and out of facilities.

What does the technology behind Serenelle look like?
We have built a single, integrated platform that holds every client record, every assessment, every coordination note, every ride, and every message — across every department. Coordinators, caregivers, drivers, and partners all work from the same source of truth. Remote monitoring and medication reminders are built in. The platform is HIPAA-compliant and audited.

What are your outcomes, and where is Serenelle headed next?
We are tracking readmissions, housing stability, medication adherence, employment, and behavioral health follow-up for every client. As a young organization, our early data is promising and we are committed to transparent reporting. Our roadmap includes expanding Vernon's Place capacity, deepening hospital partnerships, and building the coordinated care infrastructure to make Marion County a national model.

Frequently asked: Can you serve my client? How do I refer?
Yes. You can refer through our Partner Portal at serenelleliving.com, by phone, or by direct introduction to our team. We will assess the client, assign a coordinator, and begin coordination — typically within 48 hours. There is no wrong door. If you are unsure whether we can help, call us and ask.
